For screening, please provide one of the following below:

A photo of a government-issued photo ID (driver’s license or passport) showing name, birthday, and photo. Please send this directly to MeetMayaLee@protonmail.com
Employment verification (LinkedIn with clear photo or company website displaying name and photo)
2 independent provider references with email/phone number and links to website or ad
